Prepare For Your Appointment

You will need the following paperwork filled out prior to your one-on-one homeownership counseling appointment.  You need to fill out all four pages of the counseling worksheet, statement of counseling services and privacy policy and the credit release form.  Both of these forms are in Adobe Reader files.  If you don't have the software, download it free prior to attempting to access the files.  Gather your monthly bills together to assist you in filling out the worksheet. 

Although it is not necessary, it would also be helpful if you obtained and printed out your free credit report.  You are allowed one free report from each credit reporting services each year.  This site is the one set up to comply with the law.

It you are attending your counseling session in person, bring the completed counseling worksheet with you to your appointment.  If you are conducting your session by phone, please return it to our office in advance of your appointment and keep a copy for yourself to refer to during the session.  Also make sure to have your three most recent pay stubs.
